New Team Registration Information Form

Players must register 5 days in advance of playing. ALL 3 ITEMS (release of liability form,photo from new players,ID fee) must be received 5 days in advance, otherwise it'll be considered a RUSH ORDER if need the card right away.
Please refer to the "Player ID Card" link for complete instructions on registering. YOU CAN TYPE DIRECTLY INTO THE WAIVER FIELDS, BUT YOU MUST STILL PRINT IT OUT AND PHYSICALLY SIGN IT, AND THEN FAX/SNAIL-MAIL/SCAN&ATTACH IT.

2008 Release of Liability Form

If you are protesting a red card, you must file within 72 hours of the infraction. The red card fine PLUS a $10 fee is due with the form. Grievance will NOT be investigated until the fine/fee is paid. Fees will not be returned unless the card is dismissed. Click on the "Grievance" link for complete information.
