Use this form prior to your game for players that may not be eligible to play a minimum of 10 plays. This form needs to be reiviewed with the parent or gaurdian prior to the game. Reasons to use this form may be due to the players attendance or an injury to a player. Provide this form to the Field Supervisor for review prior to a game.
Use this form to record your 10 play requirement. Provide this form to your field supervisor after each game. If the filed supervisor does not want your form please retain it for your record.
Scholarships are based on a 10% enrollment process. Silver Lake holds 10% of our total registrations open to scholarship applications. IE 250 registrants will include 25 scholarships. Applications will be accepted on a first come first serve basis. Applications will be reviewed by the board and approved accordingly.
