Niskayuna Lacrosse Club Inc. Parental Release and Participant Assumption of Risk Agreement In consideration of my son's/daughter's participation, I/we hereby: 1. agree to outfit my child with equipment as dictated by the Niskayuna Lacrosse Club, Inc. (“NLC, Inc.”). 2. agree to assume all risk of personal injury, now and in the future, arising from participation in NLC, Inc., including, but not limited to, physical injury, broken bones, and contusions. 3. agree not to hold the coaches, officials, volunteers, officers or directors of NLC, Inc. or NLC, Inc. responsible for any injury sustained during participation in any phase of activities of Niskayuna Lacrosse Club, Inc. 4. agree to release and not to bring and suit, action or any other proceeding against the coaches, officials, volunteers, officers or directors of NLC, Inc., or NLC, Inc. for any injury sustained by my child caused by the ordinary negligence on the part of the coaches, officials, volunteers, officers or directors of Niskayuna Lacrosse Club, Inc. 5. agree not to bring suit against the coaches, officials, volunteers, officers and directors of NLC, Inc., or NLC, Inc., for any injuries that may be sustained by me/us or any member of my/our family as spectators of a lacrosse practice or game. 6. agree and consent, in the event I/we are not present, to any coach obtaining any and all medical care necessary to be administered to my child in the event of an injury, at my expense. 7. agree to accept any decisions made by the coaches in terminating attendance by my/our child due to unacceptable behavior and/or other violations of NLC, Inc. guidelines or policies. 8. agree to the guidelines and policies, including all codes of conduct, as may be established from time to time by NLC, Inc. 9. agree to provide transportation for my child to and from all practices and games. 10. agree and acknowledge that participating in NLC, Inc. is voluntary, but that I/we desire my child to participate despite the possible dangers and risks and despite this Release. 11. acknowledge that I/we have read, understand and agree to this Release. 12. agree that my child must be a us lacrosse member in current status and that my child, as a condition to comply with our guidelines and policies, comply with the US lacrosse guidelines and policies.

Contribution	Consider a Tax Deductible Donation, any amount small or large will help our program grow. We are a 501(C)(3) non-profit organization with a mission to promote and grow the sport of lacrosse at the youth level. We give each participant the opportunity to learn and play the game of lacrosse in a fun and competitive atmosphere with his or her peers regardless of experience or skill level. We will continue to provide these opportunities and encourage parents to be active within the club as we continue to grow.
