FLAG FOOTBALL REGISTRATION FORM

The consent form must be filled out prior to playing with the Marshwood Little Hawks Organization. A signed copy of an up to date physical must accompany this form prior to any games.

Insurance Information Form

These Code of Ethics are required to be signed by all players, coaches and parents or legal guardians prior to becoming a team member of the Marshwood Little Hawks Football Organization.
