Welcome to Middletown Youth Football and Cheerleading! Sign our guestbook! PAPERWORK TURN-IN AND EQUIPMENT ISSUE A newsletter will be sent to all registered families. This newsletter will also include any missing documents needed for registration. Required documents are the front and back of the Parent Consent/Registration Form, Physical/Medical Form, original birth certificate for new registrants, and a copy of the end of the year report card. Paperwork turn-in is July 19th, 10 am to 1 pm. Football equipment will also be issued this day but only if all required paperwork is submitted. From noon to 1 pm new orders for cheer clothing can be submitted.
Handout: 2008 Parent Information 2008 REGISTRATION REGISTRATIONS ARE STILL BEING ACCEPTED! DON'T WAIT! Early bird registration has ended. Walk-in registrations have already been held. Click on 2008 Registration on the menu on the left for instructions. You can pay registration fees online this year! A notice was mailed to all families who were registered last year. Register now to confirm a place on a team! Pay the registration, download the registration form and email or mail. There are only limited openings left on some teams. You should have Adobe Reader 8 for full use of the registration form. Click here to obtain a free download of Adobe Reader 8. Click on handout to download the registration forms. Email address and weight are mandatory fields. If you don't have an email address or are registering a cheerleader, type NA in those fields.
