Uniforms
Coaches and Team parents:
Attached is the Puma Uniform order form and summary
sheet. Please provide me immediately a uniform
coordinator's name, phone number and email address for
all further information.
Uniform samples will be available for sizing/viewing
on Saturday 5/17 from 6-8 p.m. and 6/2 during tryouts.
Please encourage your parents to check for proper
sizing. If player choses the wrong size, they are
responsible for payment of an additional size.
Please make sure you print enough copies of the order
form to pass out at your tryouts. You must download
the form to print out the pictures.
To Place Uniform Orders for your Team:
Please have each player complete the individual order
form. Make sure that the size and player number are
correct. Collect checks payable to Efingers Sporting
Goods, or Credit cards are accepted, please make sure
that all information is completed. Team
representative will be responsible if followup is
necessary.
Have team uniform coordinator collect forms and
complete the tally sheet attached.
All orders must be submitted by Sunday, June 22, 2008.
Add-on orders will be accepted until July 20, 2008.
A box will be left outside my garage door to collect
order forms.

* Order forms and Payments
* roster listing All players and their jersey #
Please note if any of this information is incomplete
order will not be submitted and will be returned to
coordinator.
