D16 Presidents Affidavit Review Meetings Summary
Affidavit Must be signed by President, Player Agent, and Manager. Number of Coaches allowable is based on number of players. Original Birth Documentation and 3 proofs of residence for each player is required. All Waivors and supporting documentation is also required. Maps are required and must be signed by League President. An extra copy of the entire affidavit and the map is required for District records. All documents below (and more) are also found at LLofA site using LINKs to Little league of America on HOMEPAGE. IF A PLAYER IS ADDED AFTER DISTRICT AFFIDAVIT REVIEW has been completed THE ORIGINAL BIRTH CERTIFICATE MUST BE CARRIED FOR THE DURATION OF TOURNAMENT PLAY.
