This form can be filled out with Acrobat Reader and printed. You cannot save this form unless you have a full version of Adobe Acrobat. Put the cursor in the field titled, "Club Name" and enter the requested info. You can tab through the form or click to enter data in any field. The, "Subtotal" field is automatically calculated from the values entered in the "No." and "Fee" fields. You must tab through the "Subtotal Fee" field for the calculation to occur. The final "Total" is also calculated. After you are done print a copy to send with your payment and a copy for your records.
