To access Online registration Forms for Southcentral District Leagues and the Commisioner's Cup Tournament click on the header of this announcement to go to the SCD Forms Center and then click on the appropriate form in the right hand Navigation area of the Forms Center screen. After you have completed the form, hit send at the bottom of the form it will automatically be sent to the appropriate commissioner or SCD Board member. Check out the SCD Calendar on the SCD website for information on deadlines.

The attached PDF can be filled out on your computer and then printed to accompany your registration or fee payment to the SCD Vice President, Joe Conte. Checks should be made out to CJSA-SCD.

Instructions for use of the Form: This form can be filled out with Acrobat Reader and printed. You cannot save this form unless you have a full version of Adobe Acrobat. Put the cursor in the field titled, "Club Name" and enter the requested info. You can tab through the form or click to enter data in any field. The, "Subtotal" field is automatically calculated from the values entered in the "No." and "Fee" fields. You must tab through the "Subtotal Fee" field for the calculation to occur. The final "Total" is also calculated. After you are done print a copy to send with your payment and a copy for your records.
