BYS Code of Conduct

This handout is to be used by girls trying out for the u12/u11 team. This form must be accompanied by the BYS and Medical Release Form.

U12-U11 Girls Registration

This registration form should be completed by players entering Boonville Junior High School in the Fall of 2008. Players entered grade 6 and interested in playing at the Junior High level should contact boys or girls Junior High coach prior to completing.

2008 Junior High Registration

This form should be completed and returned with payment to register. No child will be denied participation if registered prior to July 26. All late registrations will be placed on a waiting list.

2008 Fall Recreational Registration

This form must be completed prior to participating and is used by staff and coaches in case of emergency.
