Mandatory Background Checks

Bert Bell requires that ALL coaches submit Criminal History and Child Abuse History Clearance background checks. Links are provided below for forms to fill out.

Criminal History Background Checks can be done online, Click Here, or download the form below. The online results are instant and can be printed online.

Child Abuse History Clearance forms must be mailed along with a Money Order to the address provided on the form (Allow 2 weeks for this form to be returned).
